University Hospitals Case Medical Center

John T. Carey Special Immunology Unit Educational Objectives:
Who:

5-6 ID fellows/year, one-half day per week continuity clinic.

Goals and Objectives:

To become familiar with outpatient management of HIV disease.

Teaching methods:

Direct preceptorship with a designated attending physician.

Mix of patients, patient characteristics, clinical encounters, procedures and services: Fellows attend clinic in the Special Immunology Unit (SIU) one-half day a week.

Each fellow is paired with an attending physician to follow a cohort of patients. Patients comprise the entire spectrum of HIV disease, including new patients, early asymptomatic disease, patients stable on antiretroviral therapy, patients with opportunistic infections and patients on hospice care. Each SIU patient is cared for by an individual team including a nurse care manager, medical assistant, and the fellow and attending physician.

Additional resources available include an on site dietician and social worker. The SIU serves as the primary care provider for the majority of our patients, therefore visits include routine scheduled follow-up visits, as well as urgent visits for acute problems.

Educational Resources:

Each fellow is given a packet of pertinent articles including the latest DHHS guidelines for HIV care, as well as OI prophylaxis guidelines, information on HIV resistance testing, and other seminal review articles.

There is a monthly HIV journal club, and biweekly AIDS Research and Care Conference. Online access for literature searches and journals from the hospital Core Library is available in clinic, and the ID library upstairs contains the important AIDS journals
